Job descriptions & requirements
Responsibilities:
- S/he ensures Coordinates community-based HIV Testing Services (HTS) activities in close collaboration with the CCWs to ensure efficient and targeted case identification.
- S/he ensures index/sibling testing is offered to all the newly/old enrolled HHs on the project yet to be tested.
- S/he ensures prompt bi-directional linkage of newly diagnosed individuals to ART in collaboration with the facility enrolment staff within 24-48 hrs.
- S/he ensures 100% monthly facility service uptake for all eligible beneficiaries through tracking of the CCWs
- Immediate follow up with outcome on reported missed appointments/interrupted in treatment using tracking tools and registers by the CCWs
- S/he collaborates with linkage facilitators to share monthly eligibility line-lists of Recipients of Care (ROC) with the state office
- Work with the CCWs and LFs to ensure missed appointments (drug pick-up and VL) and IITs are returned to care
- Work with the CSO’s SIO staff to ensure prompt documentation of service provision into the data base
- S/he ensures that all Children and Adolescents Living with HIV (CALHIV) achieve and maintain viral suppression, including proper documentation of viral load results
- Collaborates with LF to ensure presumptive cases undergo geneXpert in the facility through the DOT officer to improve TB referral services and ensure TB positive patients are commenced on anti TB drugs
- Organize CBO cluster-based CAD-PDX activity for TB case detection
- Ensure enrolment into health insurance, OTZ and support disclosure practices
- Collaborate with the TBAs/PMTCT focal persons to ensure all HIV-positive pregnant adolescent/women are enrolled on OVC program and follow up on the exposed infant till final outcome
- S/he ensures that nutrition assessment is done during home visits by CCW
- S/he ensures that appropriate nutrition counselling is provided during home visits by CCW
- S/he ensures that moderately malnourished children are provided with Tom Brown
- S/he ensures that severely malnourished children are referred for appropriate health and nutritional services and treatment.
- S/he ensures that eligible children are referred by CCW for uptake of vitamin A and other micronutrients during MNCHW
- S/he will facilitate referral and uptake of nutrition emergency services
- Monthly submission of the care and support unit report to the state
- Monthly submission of Nutrition unit report to the State
- Attends regularly unit technical meetings
- Perform other additional duties assigned
Requirements:
- Minimum of 1 year of experience
- Minimum of a BSc.
