Responsibilities:
Assessment and Case Management:
- Conduct comprehensive assessments of each child's background, strengths, needs, and risks.
- Develop and implement individualized care plans in collaboration with the child, their family (if applicable), and relevant stakeholders.
- Regularly review and update care plans to reflect the evolving needs and progress of the children.
Crisis Intervention and Support:
- Provide immediate assistance and intervention in crisis situations, such as emotional distress, conflict resolution, or emergency medical needs.
- Offer counselling, emotional support, and coping strategies to help children navigate challenges and trauma.
Advocacy and Referral:
- Advocate for the rights and best interests of the children within the legal, social, and educational systems.
- Liaise with external agencies, organization's, and professionals to access resources, services, and specialized support for the children as needed.
Education and Life Skills Development:
- Facilitate educational support, including enrolment in school, tutoring, and educational workshops.
- Organize life skills training sessions to empower children with essential skills such as budgeting, communication, and conflict resolution.
Family and Community Engagement:
- Maintain regular communication with the families of the children, involving them in the care planning process and providing support and guidance as appropriate.
- Foster positive relationships with the local community to create a supportive environment for the children and to access additional resources and opportunities.
Documentation and Reporting:
- Maintain accurate and confidential records of all interactions, assessments, and interventions in accordance with organizational policies and legal requirements.
- Prepare reports, progress notes, and case summaries for internal review and external reporting purposes.
