Community Health Officer/Community Health Extended Worker (CHO/CHEW)

Responsibilities:

• Providing direct patient care, including assessing, planning, implementing, and evaluating nursing interventions.
• Administering medications and treatments according to physician orders and nursing protocols.
• Monitoring patient vital signs and response to treatment, and reporting any changes or concerns to the healthcare team.
• Collaborating with physicians and other healthcare professionals to develop and implement individualized care plans for patients.
• Educating patients and their families about health conditions, treatment options, and self-care techniques.
• Ensuring accurate and timely documentation of patient assessments, interventions, and outcomes in the medical record.
• Adhering to infection control protocols and maintaining a safe and clean patient care environment.
• Providing emotional support and comfort to patients and their families during times of illness or distress.
• Participating in interdisciplinary team meetings and contributing to the development of policies and procedures.
• Engaging in ongoing professional development activities to enhance nursing knowledge and skills.

Requirements:

• Diploma from an accredited training institution.
• Current CHPRN license as a CHO/CHEW.
• Clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Must be computer literate. Experience with EMR systems will be an added advantage.
• Ability to work effectively in a fast-paced healthcare environment.
